About this location

The Vines is located in the beautiful town of Crowborough, East Sussex. It offers residential support for 12 males and females with an acquired brain injury (ABI), traumatic brain injury (TBI) and progressive neurological conditions (PNCs). This includes Parkinson’s disease, stroke and Korsakoff's syndrome. We are on a private road, in a semi-rural location.

Our team are dedicated to promoting independence and helping every person we care for to return back into the community, where possible. We want to support our residents to achieve their life goals.

In addition, The Vines can accommodate those who have a dual diagnosis, where their primary diagnosis is an ABI, TBI or PNC. This includes people who:

  • Are on the autism spectrum
  • Have epilepsy
  • Have diabetes
  • Have dementia

We can also support people who present with mild to moderate behaviours that challenge.

About our service

Our facilities and environment

The large detached, Victorian house has lots of space and offers a relaxed, homely environment. There are 12 bedrooms split over two floors, three of which have en-suite facilities, two have their own toilet, and seven have wash basins. In addition, there are two self-contained flats. They have their own wet rooms, lounges and activities of daily living (ADL) kitchenettes.

There are also a range of spacious communal areas, including a lounge, dining room, kitchen and quiet room. The large well-maintained garden offers a tranquil environment for residents.

In summary, we offer:

We also have a large, pleasant garden, which is complete with:

We also have specialised equipment within the home, such as a bath hoist.

Our bedrooms

All of our bedrooms are comfortable, welcoming and can be personalised to individual tastes. Our bedrooms also provide wheelchair access and three have en-suite facilities, containing a wet room.

Our approach to support

Everybody who lives at The Vines has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as going out in the community, personal care, cooking and cleaning, and maintaining family relationships.

We support people in their rehabilitation journey through a variety of support techniques, some of which are delivered by our in-house multidisciplinary team (MDT), and others through a community MDT. These include:

  • Positive behaviour support (PBS)
  • Occupational therapy
  • Physiotherapy
  • Speech and language therapy
  • Psychology

In addition, we use tools such as Smart Goals to support our residents to achieve the things that are important to them. We may offer additional therapies such as pet therapy and music therapy, which our residents find enjoyable and therapeutic.

The lengths of the placements that we offer at The Vines can be transitional, medium or long-term residential, depending on the needs of each person.

Activities

Everyone who lives at The Vines has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team organise a variety of activities at the home, as well as out in the community.

We focus on supporting our residents to grow their independence. For some people, this may mean developing their daily living skills within the home. For others, this could be accessing work or educational opportunities in the local community.

Exclusion profile

  • People with active alcohol or substance misuse issues
  • People who display severe behaviours that challenge
  • Those with an active risk of absconsion

Pathways

Through our network of healthcare, residential and supported living facilities, we provide unique care pathways which help the people we support to progress towards greater independence. Every individual we support is provided with a bespoke pathway plan, tailored according to their individual needs. We aim to ensure that each person is supported to live a fulfilling and active life in the community.

We work closely with Priory Elm Park to support people in their rehabilitation journey who are ready to step down to community living.

Family and friends FAQs

How does home leave work?

Residents are support to attend family events and visit their family homes. Beforehand, a risk assessment is carried out, and the individual’s key worker may come along on the visit to help things run smoothly.

Can friends and families visit?

We welcome the loved ones of our residents to our home any time. They are also welcome to join in our activities at the home too. All we ask is that visits are planned in advance to support individual needs. Unfortunately, family members cannot stay overnight.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

We arrange more regular catch-ups either in person or via telephone, to ensure families are kept informed. We will work with you on an individual basis to work out a style of communication that suits you.

Will my loved one be able to have a phone or call me?

We have a landline and cordless phones in the home for residents to use. Some of the people staying with us are also able to have their own mobile phone.

What are the bedrooms like?

The bedrooms are furnished to suit the needs of our residents. We encourage them to personalise their rooms so they feel homely, and we can help with this.

Are external doors kept locked?

Our external doors are locked, to keep everyone safe. However, every resident will be risk assessed to determine the level of security they need. They can move around the home freely, enjoying our large garden area.

What do residents eat and how do meal times work?

Our staff support residents to choose and prepare what they want to eat. Some people need an extra hand with preparing their food or have special dietary requirements, which our staff are happy to help with.

How does laundry work?

Our team will ensure that residents always have access to clean clothes and bedding. We have a laundry room which residents make use of either independently or with the support of their key worker.

Who does the housekeeping and domiciliary tasks?

Most of our residents help to keep their rooms clean and tidy, supported by our staff team. This helps people build up their daily living skills, as well as take pride in their environment. We also have a housekeeper, who ensures the communal spaces stay clean.

Is there anything they can’t bring or have?

Residents are not allowed to bring illegal substances onto the premises. We would also ask that people do not bring high risk personal items into the house. Typically, we do not allow alcohol on-site. We may place restrictions on other items where the risks cannot be appropriately managed.

How do activities work?

Activities include both in-house activities as well as visits into the community. Their key worker may use a variety of communication tools to ensure residents can have an input into their individually tailored timetable. We seek to understand people’s hobbies and goals, so we can plan activities they find enriching.

How will residents be supported with their behavioural needs?

We focus on getting to know our residents, so we can best understand their unique needs, and put in place the support required to ensure their wellbeing. We have a psychologist and psychology assistant, who provide additional support, where required.

At The Vines, we use the PROACT-SCIPr-UK® framework, which means we focus on proactive strategies, rather than physical intervention. We require our care plans which include restrictions to be assessed frequently.

Do residents and families have an input into the care plans?

We work with social workers, family members and residents to develop care plans which are in the best interests of our residents. We use a variety of communication tools and work with health professionals to aid each resident in having a say about every aspect of their care. Care plans are regularly reviewed, and family members will be asked for their input.

What are the car parking facilities?

We have on-site parking for up to ten cars.

What is the smoking policy?

Residents can smoke by the designated smoking area in the back garden, which has a shelter. We don’t permit smoking inside the building, and this includes vapes. Residents are able to buy cigarettes and vapes at the shops, if this is what they choose.

What are your fees and how are they funded?

Our fees can vary, and are based on an assessment of each individual's needs. Many people are eligible for financial help towards the cost of support, and this funding can be accessed by contacting your local authority. Once funding has been agreed, we will work with your local care team to put together a bespoke package of care. Please note, referrals for NHS or Local Authority funded services must come from a referring organisation.
