Our approach to support

Everyone who lives at The Piers has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as daily living skills, ordering and dispensing their own medication, going out in the community, making and attending appointments, and maintaining links with family.

We have a person-centred and proactive approach to support, and also have strong links with the community learning disability team. The lengths of the placements that we offer at The Piers can be medium or long-term residential, depending on the needs of each person.

Activities

Everyone who lives at The Piers has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team organise a variety of activities at the home, as well as out in the community.

We focus on supporting our residents to grow their independence. For some people, this may mean developing their daily living skills within the home. For others, this could be accessing work or educational opportunities in the local community.

Pathways

Through our network of healthcare, residential and supported living facilities, we provide unique care pathways which help the people we support to progress towards greater independence. Every individual we support is provided with a bespoke pathway plan, tailored according to their individual needs. We aim to ensure that each person is supported to live a fulfilling and active life in the community.

Family and friends FAQs

How does home leave work?

Two of our residents regularly visit family. One spends every other weekend with their father, and another visits their mother for the weekend every six weeks.

Can friends and families visit?

Family and friends are welcome to visit any time. We just ask that they inform us in advance of any visit.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

Yes, we encourage family members to take an active part in their loved one’s care, and we are always on hand to answer any questions. We also encourage family to attend review meetings.

Will my loved one be able to have a phone or call me?

There are no restrictions regarding residents being able to contact family. We will support people to maintain links with family and friends and staff are around 24 hours a day if you wish to call.

What are the bedrooms like?

The bedrooms are all large double rooms which are fully furnished to a high standard.

Are external doors kept locked?

Yes, external doors and the gate are kept locked due to the very vulnerable nature of our residents. However, we have a large back garden that residents can freely access.

What do residents eat and how do meal times work?

We have planned menus over a 4-week period. Shopping is delivered weekly, and residents can go out with staff to the local shops to purchase anything extra they would like. Staff cook the meals, and residents are encouraged to actively take part in this. Meals are planned around variety and maintaining good health, and snacks are readily available.

How does laundry work?

Residents are supported to complete their laundry in our on-site utility room. Residents take turns to do their laundry, so clothes will not be washed together.

Who does the housekeeping and domiciliary tasks?

Staff will generally take care of the day-to-day housekeeping, but we do encourage residents to do as much as they are able to for themselves. Residents are encouraged to maintain their own bedrooms.

Is there anything they can’t bring or have?

Residents are unable to have pets on-site and we may place restrictions on some items where the risks cannot be appropriately managed, such as non-approved drugs and medications.

How do activities work?

Each resident has their own preferred activities and are supported to access activities by staff. Residents are encouraged to try new activities, which may be suggested by staff.

How will residents be supported with their behavioural needs?

All staff are trained in PBS.

Do residents and families have an input into the care plans?

Due to their disabilities, residents’ care plans are written up to a best interests framework. Family are encouraged to take an active part in reviews and other meetings.

What are the car parking facilities?

There is a large off-road car park to the front of the building.

What is the smoking policy?

The Piers is a smoke free service, however, there is a designated smoking area outside in the garden, should people wish to smoke.

What are your fees and how are they funded?

Our fees can vary, and are based on an assessment of each individual's needs. Many people are eligible for financial help towards the cost of support, and this funding can be accessed by contacting your local authority. Once funding has been agreed, we will work with your local care team to put together a bespoke package of care. Please note, referrals for NHS or Local Authority funded services must come from a referring organisation.

How to make a referral

Our service provides high quality support to people with varying levels of need. Referrals can be made through the individual’s social care team or, if relevant, their local health authority. If you are a professional looking to make a referral, please call us or fill in our enquiry form.
