Achieving positive outcomes
By encouraging people to take control and have belief in their potential, we can make positive outcomes a reality for everyone, regardless of age or presumed ability. We create pathways that support individuals through residential to community living, delivering measurable results for both local authorities and the people who use our services.
The team here at Sapphire House are supported by one of our Assessment and Transitions Directors, and our PBS team, to plan and execute successful transitions for individuals from a variety of backgrounds, including those coming from inpatient settings.
We also work closely with our colleagues at Priory Supported Living London & Home Counties to enable our residents to progress to more independent living, when they are ready to do so.
“I like living at Sapphire House as I can see my friends and I am developing my skills.”
