About this location

Parc Wern Care Home is located in the village of Ammanford, Carmarthenshire, Wales. Our home offers specialist nursing and dementia support for 59 adults, with 24-hour input from registered nurses. We can also offer end of life/palliative care and respite services.

The home is conveniently located in the semi-rural area of Penybanc. From its elevated position it enjoys extensive views of the surrounding countryside and mountains. Our goal at Parc Wern is to provide the support to our residents they need to live a happy and fulfilled life.

In addition to supporting adults with dementia, we can also assist those who have a range of other conditions, presentations and needs, who require nursing support.

About our service

Our facilities and environment

Parc Wern is a detached, single storey building. The service is divided into 3 units, all named after local areas: Pennybanc, Saron and Garnant. In addition to the accommodation, the home has a range of communal areas for residents to enjoy, including:

Residents also benefit from our tranquil garden areas, complete with:

We also have specialised equipment within the home, such as a variety of hoists.

Our bedrooms

All of our bedrooms have been designed to provide maximum comfort to the people we support. The rooms are all furnished and homely, and can be personalised to individual tastes. Each bedroom has:

Our approach to support

Everybody who lives at Parc Wern has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as personal care, mobility, eating and drinking, accessing the community, and social opportunities.

In addition to our 24/7 nursing support, we also have close links with multidisciplinary professionals, who help us ensure the health and wellbeing of the people we support. We have frequent hairdresser visits, and regular visits from a GP and dentist. We also work with additional healthcare professionals, where required.

Activities

Everyone who lives at Parc Wern has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team, led by an activities co-ordinator, organise a variety of activities at the home, as well as out in the community. This might include activities such as:

Our support staff are highly experienced in supporting older people. Our in-house team consists of:

Family and friends FAQs

How does home leave work?

We support our residents to attend family events and also home visits. We will review with the family and ensure that the visits are facilitated safely.

Can friends and families visit?

We provide an open door visiting policy and families are welcome to attend our activities offered at the service. Please do speak with the team if you have any requests.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

Yes, we strive to keep families involved in the care and support of their loved ones. We hold quarterly meetings where we discuss care, support, and health, and we can also provide catch ups more regularly if you would prefer, either face-to-face or via the telephone.

Will my loved one be able to have a phone or call me?

We will support your loved one to use their own mobile phone where required, but we also have direct incoming lines into each unit and portable phones which can be used.

What type of things are families expected to provide, and what is provided by the home?

We provide each bedroom with a bed, bedding and basic furniture. Meals and snacks are also provided as well as in-house activities. We would expect families to provide their loved ones with clothing, toiletries and any personal items for the bedroom, as detailed in our welcome guide.

What are the bedrooms like?

All bedrooms are ready and furnished, and can be personalised to meet the needs and wishes of your loved one.

Are external doors kept locked?

The service can only be accessed by a door code, we will however review access for your loved ones through the assessment process, to detail the security required to keep them safe. We do have a large garden area and courtyard which is accessible to residents and is also secure.

What do residents eat and how do meal times work?

Parc Wern has a weekly menu which changes with the seasons and is created with resident involvement, offering Welsh favourites. We provide three meals per day and an array of snacks and drinks of choice. Menus are available in easy read format, and we also cater for specific dietary requirements.

How does laundry work?

We have our own laundry facilities on-site and a dedicated team of laundry assistants who ensure that clothing and bedding is clean.

Is there anything they can’t bring or have?

We would politely request that you refrain from bringing valuables or family heirlooms into the property.

Are pets allowed?

Unfortunately we don't allow pets to live at Parc Wern.

How do activities work?

We provide a bespoke, individually tailored activity programme for each resident, delivered by our dedicated activities co-ordinator. They will organise a robust programme of activities, which will be dependent on the season and also the preferences of our residents.

Do residents and families have an input into the care plans?

As part of the initial assessment we will work with families and social care professionals to gain information to build and develop bespoke person centred support plans. Our systems for record keeping are electronic which means they are easily updated, and will be frequently reviewed to ensure they reflect each person’s needs.

What are the car parking facilities?

At Parc Wern we provide car parking facilities which are free and accessible 24 hours a day.

What is the smoking policy?

We provide a designated smoking area outside the home, and do not allow smoking inside of the building.

What are your fees and how are they funded?

Our fees can vary, and are based on an assessment of each individual's needs. We accept the following funding types: local authority funding, local authority funding and top up, NHS Continuing Healthcare (CHC) funding, and private/self-funding. Please note, referrals for NHS or local authority funded services must come from a referring organisation. We recommend contacting your local social care team to attain a referral.
