About this location

Hamilton House is located in the village of Leigh Sinton, Worcestershire. Our service offers residential support for six males and females who are autistic or have a learning disability. The home is on a quiet road in a rural area.

We are dedicated to improving the health and wellbeing of the people we support, helping them to achieve positive outcomes. Our team want to help each person we care for to have a healthy, happy and independent life, within a safe and welcoming home.

We can also support those who present behaviours that may challenge.

About our service

Our facilities and environment

We are a large detached house surrounded by extensive gardens. There are five bedrooms in the house, one of which is on the ground floor, and all of these rooms have en-suite facilities.

Residents living in the main house have access to large communal areas, including:

  • A large lounge with plenty of seating
  • Kitchen and dining room
  • Utility room

We also have a very large garden, complete with:

  • Secure fencing
  • Lawn
  • Patio
  • Communal seating
  • Summer house
  • Shelter
  • Vegetable patch

Our bedrooms

All of our bedrooms are comfortable, welcoming and can be personalised to individual tastes. In addition to the five en-suite bedrooms in the main home, there is an additional self-contained flat in an adjacent building. The flat is split across two floors, and has its own kitchen, lounge, bedroom, and en-suite bathroom with a shower.

Our approach to support

Everybody who lives at Hamilton House has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as going out in the community, personal care, cooking and cleaning, managing finances and maintaining family relationships.

Our support approaches at Hamilton House include positive behaviour support (PBS). We also have support from a local multidisciplinary team to offer other services. In addition, we use tools such as the Outcomes Star™ to support our residents to achieve their goals.

The lengths of the placements that we offer at Hamilton House can be medium or long-term residential, depending on the needs of each person.

Activities

Everyone who lives at Hamilton House has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team organise a variety of activities at the home, as well as out in the community. Our activities include:

We focus on supporting our residents to grow their independence. For some people, this may mean developing their daily living skills within the home. For others, this could be accessing work or educational opportunities in the local community.

Pathways

Through our network of healthcare, residential and supported living facilities, we provide unique care pathways which help the people we support to progress towards greater independence. Every individual we support is provided with a bespoke pathway plan, tailored according to their individual needs. We aim to ensure that each person is supported to live a fulfilling and active life in the community. At Hamilton House, we have close links with Priory Supported Living The Shires.

Family and friends FAQs

How does home leave work?

Yes, our residents can go on home leave. We are very supportive, and our staff work hard to ensure the home visit goes smoothly.

Can friends and families visit?

Residents living at Hamilton House can have relatives visit them in their home whenever. We are happy to facilitate visits.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

We can arrange the best way to stay in touch on an individual basis, either through regular calls, emails or message exchanges, or with in-person visits.

Will my loved one be able to have a phone or call me?

Yes, some of our residents have their own mobile phones. We also have a landline available for use by our residents.

What are the bedrooms like?

Bedrooms are all furnished, and residents are supported to personalise them to suit their needs and wishes. Our team are happy to support with this in any way we can, and we encourage residents to bring their own furnishings or personal items, to make their space their own.

Are external doors kept locked?

Access to grounds is through a secure key-fob operated gate, for our residents’ safety. Residents have access to all areas of the home, with care staff support.

What do residents eat and how do meal times work?

Residents are supported to choose their meals at the start of the week, and are encouraged to play a role in food shopping and meal preparation, to build their skills in these areas. They can also choose to eat in their bedroom or in the dining area.

How does laundry work?

Residents are supported to do their own laundry as and when needed. We will always ensure residents have access to clean bedding and clothing.

Who does the housekeeping and domiciliary tasks?

Residents are encouraged to be involved in all aspects of their life with emphasis on daily living skills like cleaning their rooms. Residents are supported in making sure their own spaces are always kept clean and tidy, and our team can help where a person is not able.

Is there anything they can’t bring or have?

We would ask that residents do not bring valuables such as family heirlooms to our home, as we cannot guarantee their safety. Residents have access to limited storage in their own bedrooms. We may place restrictions on some items where the risks cannot be appropriately managed.

How do activities work?

Everyone we care for has a tailored activities planner, which will include both in-house activities as well as visits into the community. Their key worker may use a variety of communication tools to ensure the resident can have an input into the planning of activities. Our activities include things like: swimming, fishing, cinema, dog walking, barbecue, board games, quiz nights, pub meals, work placements and community volunteering, and arts and crafts.

How will residents be supported with their behavioural needs?

Hamilton House uses the PROACT-SCIPr-UK® framework, which means we focus on proactive strategies, rather than physical intervention. We follow a PBS approach, facilitated by our practitioner, whereby we identify which areas of support will be most helpful to each individual, and will work with those that we support to ensure that their needs are met.

Do residents and families have an input into the care plans?

Everyone who lives here is involved in making decisions about their day-to-day care through regular key worker meetings, ‘Our Voice’ home meetings, and Regional ‘Our Voice’ meetings. All residents also have access to an independent advocacy service. Families may be involved in the care plan creation and review process, where this is in line with a resident’s wishes or best interests.

What are the car parking facilities?

We have on-site parking.

What is the smoking policy?

No smoking is allowed in the property, we have a designated area for residents to smoke or to vape.

What are your fees and how are they funded?

Our fees can vary and are based on an assessment of each individual's needs. Many people are eligible for financial help towards the cost of support, and this funding can be accessed by contacting your local authority. Once funding has been agreed, we will work with your local care team to put together a bespoke package of care. Please note, referrals for NHS or Local Authority funded services must come from a referring organisation.
