Our dedicated leadership team at Desart Mews will work with individuals, their social workers and existing placements, to carry out rigorous assessments and carefully plan transitions to ensure placements are successful.
This ‘robust’ service brings together the best qualities of a ‘local’ service, with the support and expertise of an established and well-respected service provider.
Our aim is to develop life and community skills, enabling the people we support to successfully live in their own home in the future, where possible.
The service is close to local amenities and public transport, offering many outdoor activities nearby for individuals to enjoy. This includes horse riding, outdoor gyms, walking trails, leisure centres, ten-pin bowling and theatres, as well as a number of major shopping malls with a diverse range of shops, restaurants and cafes.
Achieving positive outcomes
By encouraging people to take control and have belief in their potential, we can make positive outcomes a reality for everyone, regardless of age or presumed ability.
We create pathways that support individuals through residential to community living, delivering measurable results for both NHS Trust funders/commissioners and the people who use our services. This, paired with our personalisation agenda, enables us to help the people we support to lead fulfilled, independent lives. By working together, we can make a real and lasting difference.
