Conquest House

The support at Conquest House is delivered in partnership with a professional multidisciplinary team, committed to providing quality care focused on the needs of the individual. We believe in a personalised approach and everyone living at Conquest House has their own person-centred plan which is reviewed regularly.

Our aim is to provide the highest quality support enabling individuals to build on skills, live more independently, and reach their goals.

We also believe that individual choice is paramount and each person we support is actively encouraged to make informed choices and maintain independence. A key worker is assigned to each person ensuring a holistic approach to individual care. We encourage individuals and families to actively participate in the planning of support, which is facilitated through individual/relatives meeting where information is provided.

Conquest House work closely with specialist local services such as:

• The community learning disability team
• Community nurses
• Psychologists
• Speech therapists
• Local health centres
• Dentists
• Chiropodists

Conquest House has been tastefully decorated throughout, with decor of each bedroom tailored to meet the requirements and needs of each person. Each individual is encouraged to personalise their own space to create an area where they feel comfortable, relaxed and safe.

Conquest House facilities include:

• Communal Dining and kitchen space
• Utility room
• Tranquil, secluded garden
• Private transport

Everyone has access to the kitchen and can make themselves teas, coffees or cold drinks.

Conquest House is situated in Farcet, Peterborough with easy access to local amenities. The service has its own private transport and is situated close to local transport networks such as bus and train services to help individuals travel within the area and further afield, enabling them to visit local places of interest, day services and resource centres.

In addition, the service supports residents to access educational opportunities at the local college, which offers courses such as music, communication, dancing, speech, cooking, arts and gardening.

Each resident at Conquest House is supported with personalised activity programmes. The staff organise a wide range of daily activities both in and out of the service which are planned in conjunction with the wishes of the people living here. These include:

• Swimming
• Horse riding
• Bowling
• Shopping
• Cinema trips
• Outings to pubs

Conquest House may be suitable for those who meet the following admissions criteria:

• Adults aged 18 years and over
• Individuals with autism
• Individuals with learning disabilities
• Individuals with mental health conditions
• Individuals who may also present with challenging behaviour

Our service provides high quality care and support to people with varying levels of need. Referrals can be made through:

• The individual’s social services department management team or, if relevant, their Local Health Authority
• By the individual (or an appropriate adult acting on their behalf) who wishes to use direct payment, individual budget or who is self-financing

There are several different ways in which a Priory Adult Care service such as Conquest House can be funded. This includes:

Local Authority funding

This is where the Local Authority (typically the local Council) agrees an amount to fully fund a person’s support, whether it is in a residential placement or as part of a supported living package.

The funding package is agreed following an assessment of the person’s needs and covers both accommodation and support.

Joint funding between the Local Authority and NHS

This is where an individual has a Continuing Healthcare (CHC) assessment. In this instance, funding will be split between the Local Authority and the NHS.

The funding package is agreed following joint assessments of the person’s needs and healthcare needs. The Local Authority element of the funding will cover both accommodation and support, the NHS assessment will cover the continuing healthcare needs.

NHS Funding

It is possible that an individual’s health needs are such that the NHS will fully fund a person’s support package.

Individual/Personal Budget

This is where the Local Authority allocates a specific amount of money for a person’s support needs. The amount is determined by an assessment of the person’s needs and means. You will also need to agree a support plan to show how and where the money will be spent, whether you decide for the Local Authority to choose services for you or if you prefer to have direct control of the budget yourself – e.g. a direct payment.

Direct Payments

This is where you receive an amount directly from the Local Authority and can choose how to spend the money on your support. Choosing a direct payment gives you maximum flexibility with your support package but does mean you have to manage the money yourself.

Fees are calculated based on the assessed needs of an individual. Each fee will include a staffing element, based on the hours of support assessed and required. In some instances, the staff support may be shared with other individuals, and in this situation the individual would only pay for the share of staff time.
