About this location

Located in Trentham in Stoke-on-Trent, Collinson Court offers residential support for up to nine males and females who are autistic. We are in a residential area, off the main road.

Our team are dedicated to providing the skills needed for our residents to live a healthier and happier life back in the community.

We can also support those who have:

  • A learning disability
  • Behaviours that challenge
  • Mental health needs
  • Difficulties with communication
  • Epilepsy

About our service

Our facilities and environment

Collinson Court is a purpose-built building with nine bedrooms, all on the ground floor. The home has a variety of communal spaces, including:

  • A large lounge with plenty of seating
  • Kitchen and dining room
  • Sensory room
  • Communal bathroom

We also have our own garden, complete with:

  • Secure fencing
  • Lawn
  • Patio
  • Sensory garden

In addition, we have three site vehicles to help us to meet the needs of the people we support.

Our bedrooms

Residents live in eight flats, one of which has two bedrooms. The two-bedroom flat has its own kitchenette, communal lounge area and a bathroom with a bath. All the single flats have en-suite facilities, activities of daily living (ADL) kitchenettes and a lounge. The bathrooms have either a shower or a wet room.

The flats each have their own garden space. All of our bedrooms are comfortable, welcoming and can be personalised to individual tastes. In summary, the flats have:

  • ADL kitchenettes
  • Wheelchair access
  • En-suite facilities, containing either a shower, a bath or a wet room

Our approach to support

Everybody who lives at Collinson Court has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as going out in the community, personal care, cooking and cleaning, managing finances and maintaining family relationships.

Our support approaches at Collinson Court include:

  • Positive behaviour support (PBS)
  • Physiotherapy
  • Speech and language therapy

PBS is delivered through an on-site practitioner. We also have support from a local multidisciplinary team to offer other services. In addition, we use tools such as the Outcomes Star™ and Active Support to support our residents to achieve their goals.

The lengths of the placements that we offer at Collinson Court can be medium or long-term residential, depending on the needs of each person.

Activities

Everyone who lives at Collinson Court has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team organise a variety of activities at the home, as well as out in the community.

We focus on supporting our residents to grow their independence. For some people, this may mean developing their daily living skills within the home. For others, this could be accessing work or educational opportunities in the local community.

Pathways

Through our network of healthcare, residential and supported living facilities, we provide unique care pathways which help the people we support to progress towards greater independence. Every individual we support is provided with a bespoke pathway plan, tailored according to their individual needs. We aim to ensure that each person is supported to live a fulfilling and active life in the community.

Here at Collinson Court we work closely with Priory Supported Living West Midlands to support people to move into their own homes in the community.

A message from our site leader

We would work with the families of our residents to ensure the move into our home is a smooth one. They are welcome to visit beforehand to ensure that everything in their loved ones’ room and garden is in place. We introduce them to our staff so they can put a face to the name.

Family and friends FAQs

How does home leave work?

We encourage our residents to maintain family and friends’ relationships. The staff member supporting the resident can either go with them on their home leave, or remain at the site and can be available should family require support; this is all tailored to the resident and their families. We can also introduce a pathway of staff supporting home visits with a view of reducing the support if this is something that our resident and families want. We will also give a clear handover of what they have done before their home visit, so there is clear communication between the resident and families.

Can friends and families visit?

Yes, we welcome families to visit their loved ones. We invite family members to events at the services, ensuring that there is plenty of notice, so they can make arrangements to come. Regular family visits are usually planned, so staff know how best to support their loved one in getting ready and that the visit is communicated to them.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

Yes, we provide regular family updates via email along with pictures of what their loved one has done. If there is an emergency family will be contacted with full details of how their loved one is. We regularly review care plans and wish for families to be a part of this process, which can include regular catch-ups. If families wish to have a catch-up earlier than one that is scheduled, this can also be arranged.

Will my loved one be able to have a phone or call me?

Yes, we have three phones available that can be used, and they can be taken to any part of the building. These can be used by residents that we support to make regular contact with families. In addition, we can support our residents to use a mobile phone.

What are the bedrooms like?

Our bedrooms are fully furnished, but if a resident wishes to buy something specific for their bedroom this can be accommodated. We also support residents to redecorate their room to suit their wishes.

Are external doors kept locked?

We do have locked gates and you require key fob access to get around the building. Our residents have access to their own gardens situated outside their apartments, which they can access safely.

What do residents eat and how do meal times work?

Residents are supported to choose what they wish to eat at mealtimes. They are also supported in preparing meals where possible to promote independence. If there are any dietary requirements, advice will be sought so residents get the support they need in this area.

How does laundry work?

We do have laundry facilities where residents can take their own laundry to be washed and dried. Residents also have access to clean bedding of their choice.

Who does the housekeeping and domiciliary tasks?

Residents are encouraged to complete daily cleaning tasks to support their independence. We also have a housekeeper on site that supports with communal areas and deep cleans of the building.

Is there anything they can’t bring or have?

We would ask that residents do not bring anything valuable. We may place restrictions on some items where the risks cannot be appropriately managed, such as lighters.

How do activities work?

We have an activities co-ordinator who runs in-house and community-based activities. Residents can choose what they wish to complete and will be supported throughout the activities. We have four vehicles that residents have access to. Should residents wish to go on holiday with family or with staff, this can be accommodated.

How will residents be supported with their behavioural needs?

We follow a PBS approach, facilitated by our practitioner. This means we put in place strategies to meet residents’ needs, following bespoke PBS plans. We also use the PROACT-SCIPr-UK® framework, which means we focus on proactive strategies, rather than physical intervention. We require our care plans which include restrictions to be assessed frequently, for example procedures for interventions such as pro re nata (PRN) medication are regularly reviewed and removed, where possible.

Do residents and families have an input into the care plans?

Yes, we work with family members, social workers, learning disability nurses to develop care plans, and they are reviewed in the best interests of the resident. Where possible, we will include the person we support, to have an input in their support plans.

What are the car parking facilities?

We have a car park at the front of the home and there is another car park at the rear.

What is the smoking policy?

We do not permit smoking in the building. Should the people we support smoke, they would be able to do this in the back of their garden.

What are your fees and how are they funded?

Our fees can vary and are based on an assessment of each individual's needs. Many people are eligible for financial help towards the cost of support, and this funding can be accessed by contacting your local authority. Once funding has been agreed, we will work with your local care team to put together a bespoke package of care. Please note, referrals for NHS or Local Authority funded services must come from a referring organisation.
