About this location

Autumn Leaf House is located in Castle Bromwich, Solihull. Our home offers residential support for eight males and females who are autistic. We are on a quiet street, near the town centre.

Our team are dedicated to improving the quality of life of the people we care for by setting achievable goals, to meet their individual needs and provide meaningful relationships and activities.

We can also support those who have:

About our service

Our facilities and environment

Our large home creates a low stimulus, homely and relaxed environment, whilst offering robust furnishings, serving the needs of the people we care for. We have a beautiful, landscaped garden that is safe and private.

In summary, we offer:

  • A large lounge with plenty of seating

We also have a very large garden, complete with:

  • Secure fencing
  • Lawn
  • Communal seating

In addition, we have a mobility-adapted minibus to help us to meet the needs of the people we support.

Our bedrooms

All of our bedrooms are comfortable, welcoming and can be personalised to individual tastes. There are eight bedrooms in total, seven of which have en-suite facilities, with either a bath or a shower. Four of the rooms are on the ground floor. There is also a self-contained flat.

Our approach to support

Everybody who lives at Autumn Leaf House has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as going out in the community, personal care, cooking and cleaning, managing finances and maintaining family relationships.

Our support approaches at Autumn Leaf House include positive behaviour support (PBS) and speech and language therapy. PBS is delivered through our on-site practitioner, and we also have support from a local multidisciplinary team. In addition, we use tools such as the Outcomes Star™ to support our residents to achieve their goals.

The lengths of the placements that we offer at Autumn Leaf House can be medium or long-term residential, depending on the needs of each person.

Activities

Everyone who lives at Autumn Leaf House has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team organise a variety of activities at the home, as well as out in the community, they can range from walks to local shops, to holidays and concerts.

We focus on supporting our residents to grow their independence. For some people, this may mean developing their daily living skills within the home. For others, this could be accessing work or educational opportunities in the local community.

Through our network of healthcare, residential and supported living facilities, we provide unique care pathways which help the people we support to progress towards greater independence. Every individual we support is provided with a bespoke pathway plan, tailored according to their individual needs. We aim to ensure that each person is supported to live a fulfilling and active life in the community.

Here at Autumn Leaf House we work closely with Priory Supported Living West Midlands to support people to progress to their own tenancies, when they are ready.

Family and friends FAQs

How does home leave work?

We support our residents who visit family homes weekly. We will work with the resident and their relatives to ensure possible risks are managed and that everyone is supported.

Can friends and families visit?

Yes, family and friends are welcome to visit within a reasonable time of the day. We hold themed events and celebrations at the home, which loved ones are invited to attend.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

Yes, we maintain contact with families, and can arrange catch-ups to update them on the resident’s support, any changes, and what they have been getting up to. This could be in person, via telephone, or message.

Will my loved one be able to have a phone or call me?

Yes, everyone who stays with us are supported to call their loved ones via our telephone. We can also support video calls.

What are the bedrooms like?

All bedrooms are fully furnished and residents are supported to personalise them to suit their needs and wishes. The rooms have robust, low arousal furniture, this has been purchased to meet the needs of residents and thus cannot be removed. People are however welcome to bring their own furnishings and personal items.

Are external doors kept locked?

Yes, the gates at the front of property and the front door are locked, due to our central location. There are keypads for the front door, foyer area and kitchen. Bedrooms are only locked where necessary, we currently have one resident with an electronic lock. Security measures are put in place following robust risk assessment processes, to protect the safety of those who live here.

What do residents eat and how do meal times work?

There are no set mealtimes. Residents who are able will help prepare meals, staff will also prepare and cook main meals for all the residents. We do a seasonal menu planner to help with shopping and choice of options, this is done with resident input, but this is not set in stone. There is a communal dining room, they also have their own dining table in their room if they wish to eat in their flats. We cater for each individual needs, for example, we currently support residents who eat Halal food. Halal food has a designated fridge.

How does laundry work?

Residents are given help to do their own laundry, as part of promoting independence. Our staff are on hand to offer support if and when it is required.

Who does the housekeeping and domiciliary tasks?

Staff support residents to clean their rooms and complete daily living tasks, communal areas are cleaned and maintained by support staff and deep cleaned by the night staff. Staff support residents according to their abilities to promote independence and work proactively to skill teach and to improve quality of life.

Is there anything they can’t bring or have?

Anything valuable will be kept in locked safe in the manager’s office and recorded, we do advise that family keep hold of any valuables such as family heirlooms, as we cannot guarantee their safety. We may place restrictions on some items where the risks cannot be appropriately managed, such as non-approved drugs and medications, solvent-based products and lighters.

How do activities work?

Our activities co-ordinator plans and schedules activities to meet each individual needs, they can range from walks to local shops to holidays and concerts. We will tailor activities to residents’ preferences, hobbies and goals, and even support people to try new things.

How will residents be supported with their behavioural needs?

Our home is supported by a PBS practitioner, who works in-house with residents to develop behaviour support plans. We also use the PROACT-SCIPr-UK® framework, which means we advocate least restrictive practice. We require our care plans which include restrictions to be assessed frequently, for example procedures for interventions such as pro re nata (PRN) medication are regularly reviewed and removed, where possible.

Do residents and families have an input into the care plans?

Yes, we work with social workers, family members and residents to develop care plans which are in the best interests of our residents, through 'Our Voice' resident meetings and key worker sessions. We use a variety of communication tools such as pictorial and social stories, and work with health professionals to aid each resident in having a say about every aspect of their care. Care plans are regularly reviewed, and family members are asked for their input.

What are the car parking facilities?

We have on-site parking here at Autumn Leaf House.

What is the smoking policy?

We have a designated smoking areas for residents to smoke or to vape. We do not permit smoking inside the house, for health and safety reasons.

What are your fees and how are they funded?

Our fees can vary and are based on an assessment of each individual's needs. Many people are eligible for financial help towards the cost of support, and this funding can be accessed by contacting your local authority. Once funding has been agreed, we will work with your local care team to put together a bespoke package of care. Please note, referrals for NHS or Local Authority funded services must come from a referring organisation.
