About this location

Abbey Court Care Home is located in Bourne, Lincolnshire. Our home offers specialist nursing and dementia support for 88 adults, with 24-hour input from registered nurses. We can also offer end of life/palliative care and respite services.

Our home offers a specialised Dementia Plus service, which supports people who have dementia who may have complex support needs. Our goal at Abbey Court is to provide personalised care and support to the people who live here, so they can live happy and fulfilled lives.

In addition to supporting adults with dementia, we can also assist those who have a multitude of other conditions, presentations and needs, including those with medical needs requiring nursing care.

About our service

Our facilities and environment

Abbey Court is a modern, purpose-built home, comprised of four units split across two floors. The ground floor accommodates frail elderly residents requiring general nursing care and the first floor is for residents requiring dementia nursing care. The units are:

  • Jamine – a ground floor nursing unit
  • Sunflower – a ground floor residential unit
  • Bluebell – a first floor dementia unit
  • Forget Me Not – a first floor dementia unit


In addition to the accommodation, we also have communal areas, such as:

  • A large lounge with plenty of seating
  • Dining room
  • TV lounge

Residents also benefit from our spacious garden areas, complete with:

  • Secure fencing
  • Lawn
  • Patio area with furniture
  • Wheelchair access
  • Flower bed

We also have specialised equipment within the home, such as a lift to all floors and our own minibus to support our residents with their transport.

Our bedrooms

All of our bedrooms have been designed to provide maximum comfort to the people we support. The rooms are all furnished and homely, and can be personalised to individual tastes. Each bedroom has:

Our approach to support

Everybody who lives at Abbey Court has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as personal care, mobility, eating and drinking, accessing the community, and enjoying social opportunities.

At Abbey Court, we offer a full 24/7 nursing service and a specialised Dementia Plus service. In addition, we have close links with multidisciplinary professionals, who help us ensure the health and wellbeing of the people we support. The home has a visiting hairdresser, regular hearing and optician checks, and access to a GP and a dentist.

Activities

Everyone who lives at Abbey Court has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team organise a variety of activities at the home, as well as out in the community. This might include daily activities such as:

  • Reminiscence
  • Trips out into the community

The home is unable to support people with severe behaviours that challenge.

Our support staff are highly experienced in supporting older people. Our in-house team consists of:

The team receive specialist staff training, designed to meet the needs of each individual we support. This includes training in specialist dementia care.

Family and friends FAQs

How does home leave work?

We support our residents to attend family events and visit home. We will work with the resident and their relatives to ensure risks are managed and that everyone is supported.

Can friends and families visit?

We have an open door policy, and family and friends are welcome to visit and participate in activities at the service, 7 days a week. Unfortunately we cannot cater for visitors to stay overnight.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

Yes - we strive to keep the loved ones of our residents involved in their care. We will arrange regular meetings with families to ensure they are kept informed.

Will my loved one be able to have a phone or call me?

Our home has a phone line, which can be used by residents at any time. In addition, we can support our residents to get and use their own mobile phone.

What type of things are families expected to provide, and what is provided by the home?

Every bedroom will have a bed, bedding, and basic furniture, such as a chest of drawers or wardrobe, and bedside table. Meals and snacks are provided by our catering team. We expect families to provide clothing for their loved one, and room items such as additional décor or a television set.

What are the bedrooms like?

Bedrooms all have en-suite facilities and come furnished, and residents are supported to personalise them to suit their needs and wishes. However, some of the furniture has been purchased to suit the needs of the service specification and some of it is fitted, and therefore cannot be removed.

Are external doors kept locked?

Yes, external doors are kept locked to keep everyone safe. We do have a secure garden and courtyard area which residents can enjoy freely.

What do residents eat and how do meal times work?

We have a catering team who prepare meals three times a day, and snacks are available at all times. Residents are supported to choose what they want to eat at mealtime, and there is always a choice available, as well as easy-read menus. Our team will also seek guidance from family members and nutritionists to ensure that special dietary requirements are catered for.

How does laundry work?

Our team will ensure that residents always have access to clean clothes and bedding. We have industrial laundry facilities on-site, which our team will make use of on behalf of our residents.

Is there anything they can’t bring or have?

We would ask that residents do not bring valuables, such as family heirlooms, as we would not want these to get lost. We may place restrictions on some items where the risks cannot be appropriately managed, such as scissors.

Are pets allowed?

No, unfortunately we cannot support live-in pets.

How do activities work?

Every resident has an individual tailored activities planner. Our dedicated activities co-ordinator organises a variety of activities, with input from residents. This includes both in-house activities as well as visits outside of the home.

Do residents and families have an input into the care plans?

We work with social workers, family members and residents to develop care plans which are in the best interests of our residents. Care plans are regularly reviewed, and family members will be asked for their input.

What are the car parking facilities?

Abbey Court has car parking facilities.

What is the smoking policy?

Residents can smoke by the designated smoking area outside. We don’t permit smoking inside the building, and this includes vapes.

What are your fees and how are they funded?

Our fees can vary, and are based on an assessment of each individual's needs. We accept the following funding types: local authority funding, local authority funding and top up, NHS Continuing Healthcare (CHC) funding, and private/self-funding. Please note, referrals for NHS or local authority funded services must come from a referring organisation. We recommend contacting your local social care team to attain a referral.
