Our team are dedicated to improving the health, happiness and wellbeing of the people we support. We are focused on building positive outcomes for everyone. Some of our residents see this as their home for life, whereas others aspire to move on to more independent living, which we support and encourage.

About our service

Our facilities and environment

82 Park Street is a large, semi-detached Victorian property, set over three storeys. It is situated on a residential street, close to the town centre and local amenities. We offer four en-suite bedrooms, located over two floors, as well as spacious living areas.

We offer access to a church hall, next to one of our nearby sister homes, The Old Rectory, that provides additional space and activity rooms such as a sensory room, music area, arts and crafts facilities, and a kitchen. We also have an allotment with chickens.

Our bedrooms

All of our bedrooms are comfortable, welcoming and can be personalised to individual tastes. Our bedrooms have en-suite facilities, containing either a shower, a bath or a wet room.

Our approach to support

Everyone who lives at 82 Park Street has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as going out in the community, maintaining contact with family and friends, social opportunities, holidays, and real work opportunities.

We also have support from a community multidisciplinary team (MDT), including:

  • Positive behaviour support (PBS)
  • Psychology
  • Psychiatry

In addition, we use tools such as the Outcomes Star™ to support our residents to achieve their goals. The lengths of the placements that we offer at 82 Park Street can be transitional, short-term/respite, medium or long-term residential, depending on the needs of each person.

Activities

Everyone who lives at 82 Park Street has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team organise a variety of activities at the home, as well as out in the community.

We focus on supporting our residents to grow their independence. For some people, this may mean developing their daily living skills within the home. For others, this could be accessing work or educational opportunities in the local community.

Through our network of healthcare, residential and supported living facilities, we provide unique care pathways which help the people we support to progress towards greater independence. Every individual we support is provided with a bespoke pathway plan, tailored according to their individual needs.

We aim to ensure that each person is supported to live a fulfilling and active life in the community. Our team will support people at 82 Park Street to move into their own homes in the community, when they are ready.

Family and friends FAQs

How does home leave work?

We will always do whatever we can to support people to keep in touch with their families. We can facilitate home visits, meeting up in the community and regular phone calls.

Can friends and families visit?

Family and friends are welcome to visit our home whenever they wish. Visits are usually planned in advance in case their loved one is involved in activities or out in the community. We also have regular events at the home that families are invited to come along to.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

We strive to keep families up to date with their loved one’s progress. Key workers hold a monthly key worker meeting with each of our residents, and we can share the outcome of these with families, where requested. We can also arrange more regular catch-ups, either in person or via telephone, to ensure families are kept informed.

Will my loved one be able to have a phone or call me?

Our home has a phone line, and there are cordless phones at various points around the property which can be used at any time, in line with residents’ support plans. In addition, we can support people to get their own mobile phone, where appropriate.

What are the bedrooms like?

Our bedrooms can be personalised to suit the needs and wishes of the people staying with us. This means they can also bring in their own furniture if they wish, or we can supply items for them.

Are external doors kept locked?

Due to the nature of the service, our external doors and gates are locked. We have a large garden area where residents can freely exit and enter the building. Everyone has different security needs - some people have their own key to the front door, whilst others are supported by staff in this area.

What do residents eat and how do meal times work?

We have a four-week rolling menu that is reviewed seasonally and offers a choice of meals each day. We encourage and support people to choose meals to put on the menu, whilst ensuring that there is a variety of fresh food and menu choices are well balanced nutritionally.

How does laundry work?

We have a utility room with laundry facilities on-site. Our team will always ensure that our residents have access to clean clothes and bedding. Residents can wash their own clothes if they are able to, or staff can support them with this.

Who does the housekeeping and domiciliary tasks?

We encourage residents to participate in these tasks if they are able to, or our team can support them where necessary.

Is there anything they can’t bring or have?

We may place restrictions on some items, such as non-approved drugs and medications, solvent-based products and lighters. This is because the associated risks cannot be appropriately managed.

How do activities work?

We support and encourage people to take part in meaningful and regular activities, both in house and in the community. We support people to develop an activity planner if this is appropriate. Our support staff join in the activities too in order to ensure inclusivity and so they can offer additional support if needed.

How will residents be supported with their behavioural needs?

We use the PROACT-SCIPr-UK® framework, which means we focus on proactive strategies, rather than physical intervention. We require our care plans which include restrictions to be assessed frequently, for example procedures for interventions such as pro re nata (PRN) medication are regularly reviewed and removed, where possible.

Do residents and families have an input into the care plans?

We work with social workers, family members and the people we support to develop support plans which are in the best interests of individuals. We use a variety of communication tools and work with health professionals to aid each resident in having a say about every aspect of their care.

Support plans are regularly reviewed and updated, and family members will be asked for their input.

What are the car parking facilities?

There is on-street parking at the front of the home and some off-street parking at the rear.

What is the smoking policy?

People who live at 82 Park Street can smoke by the designated smoking area in the garden, which has a shelter. We don’t permit smoking inside the building, and this includes vapes.

What are your fees and how are they funded?

Our fees can vary and are based on an assessment of each individual's needs. Many people are eligible for financial help towards the cost of support, and this funding can be accessed by contacting your local authority. Once funding has been agreed, we will work with your local care team to put together a bespoke package of care. Please note, referrals for NHS or local authority funded services must come from a referring organisation.
