Working in partnership to support older people

Claremount House has been working with NHS Calderdale since the home first opened. The home manager and their team of dedicated staff are dedicated to improving the lives of the residents in their care. The partnership between Claremount House and NHS Calderdale enables a more person-centred service through a good continuation of care.

The aim of the close working partnership is to ensure people are discharged from hospital at the earliest possible time and moved into a home that can meet their individual needs. The NHS Calderdale Mental Health team regularly visit Claremount House and have a good understanding of the services on offer and the expertise of the care team.

Once someone has been placed into Claremount House by the Community Mental Health Team, they are visited regularly by the team leader, local psychiatrists, physiotherapists and any other professionals involved in the assessment of needs. This assessment is then given to Claremount House who can tailor the care to the needs of the individual.
