About this location

York Road is located in the village of Birkdale in Southport, Merseyside. It offers residential support for five males and females who have a learning disability or are on the autism spectrum.

Our team are dedicated to providing the people we care for with the skills needed to live a healthier and happier life back in the community.

We can also support those who have:

About our service

Our facilities and environment

The service is set in a large Victorian property on a quiet road, with the accommodation set over two floors in a warm and homely environment. The ground floor has two large living areas and a spacious shared kitchen and dining room.

The five bedrooms are split across the two upper floors with a shared bathroom on each floor. We also have a large garden at the back of the house.

Our bedrooms

All of our bedrooms are comfortable, welcoming and can be personalised to individual tastes. Our bedrooms have robust, low arousal furniture.

Our approach to support

Everybody who lives at York Road has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as going out in the community, personal care, cooking and cleaning, and maintaining family relationships.

Our support approaches at York Road include positive behaviour support (PBS) and speech and language therapy (SaLT). In addition, we use tools such as the Outcomes Star™ to support our residents to achieve their goals.

The lengths of the placements that we offer at York Road can be medium or long-term residential, depending on the needs of each person.

Activities

Everyone who lives at York Road has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team organise a variety of activities at the home, as well as out in the community.

We focus on supporting our residents to grow their independence. For some people, this may mean developing their daily living skills within the home. For others, this could be accessing work or educational opportunities in the local community.

Pathways

Through our network of healthcare, residential and supported living facilities, we provide unique care pathways which help the people we support to progress towards greater independence. Every individual we support is provided with a bespoke pathway plan, tailored according to their individual needs. We aim to ensure that each person is supported to live a fulfilling and active life in the community.

We can support people to move into their own homes in the community through close links with a Priory Supported Living Lancashire service,  Ryelands Crescent in Preston.

Family and friends FAQs

How does home leave work?

We support the people we care for to keep in touch with their loved ones by facilitating home visits, family days out and events. We ensure that where possible, we fully risk manage any potential risks by closely working with the relatives and loved ones.

Can friends and families visit?

York Road welcome family and friends to come and spend time with their loved ones. We can facilitate both indoor and outdoor visits, our lovely back garden is perfect for catching some sun in the summer. Unfortunately, we cannot cater for overnight stays.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

Yes, we strive to keep the loved ones of our residents involved in their care. On a regular basis, families are invited to a meeting at the home, where they will meet with the residents’ support team and hear about recent happenings at the home. We can also arrange more regular catch-ups either in person or via telephone, to ensure families are kept informed.

Will my loved one be able to have a phone or call me?

Everyone who stays with us are supported to call their loved ones, via phone or video call. We have multiple cordless phones at different points around the house, which can be accessed by the people that we support at all times. Some residents choose to have their own mobile phones and our staff support them to use these as required.

What are the bedrooms like?

Bedrooms are all furnished and residents are supported to personalise them to suit their needs and wishes. All our residents decide the type of furniture they would like to have in their rooms and choose the colours and themes for wall decorations.

Are external doors kept locked?

Due to the nature of our support, all our external doors remain locked during the day and night. However, all of our residents have free access to our garden space.

What do residents eat and how do meal times work?

All of our residents meet at the end of each week and supported by our staff, they choose their preferred meals and create a food menu for the week ahead. Residents are encouraged to follow any specific dietary requirements and families are asked for advice when it comes to preferred meals.

How does laundry work?

Residents are given help to do their own laundry, as part of promoting independence, using our in-house laundry facilities. Our staff are on hand to offer support if and when it is required.

Who does the housekeeping and domiciliary tasks?

The people that we support take pride in looking after their environment. Staff support them to complete household tasks, from cleaning their own bedrooms to maintain a good level of cleanliness throughout all communal areas.

Is there anything they can’t bring or have?

Residents are not allowed to bring illegal substances onto the premises. We would also ask that people do not bring high risk personal items into the house.

How do activities work?

All the people that we care for have a person-centred support plan which includes all their preferred activities. Our key workers support all them to design a weekly tailored activity planner. Our support workers assess all the associated risks and implement all required measures, so residents are able to safely enjoy their chosen activities.

How will residents be supported with their behavioural needs?

Our staff team have a positive approach when it comes to managing behaviours that may challenge. All agreed interventions and practices are well described within each individual's support plan. External professionals and families are involved in prescribing the correct and safe practices. Additional measures such as pro re nata (PRN) medications may be required, although we aim to reduce restrictive practices, and will regularly re-assess and remove interventions, where possible.

Do residents and families have an input into the care plans?

Yes, we encourage families and extended support providers to be involved with the care plans. We use a variety of communication tools to work collaboratively with the person we care for to co-produce the plans, so they are tailored to their needs. We value this important input from others where appropriate.

What are the car parking facilities?

There is off-road parking for up to two cars. Additionally, there is free parking on the road.

What is the smoking policy?

There is a designated area for residents to smoke or to vape in our back garden. We support the people who live here to purchase cigarettes at the shop, if this is what they choose. However, we do promote healthy lifestyles, and encourage residents to engage with smoking cessation programmes.

What are your fees and how are they funded?

Our fees can vary, and are based on an assessment of each individual's needs. Many people are eligible for financial help towards the cost of support, and this funding can be accessed by contacting your local authority. Once funding has been agreed, we will work with your local care team to put together a bespoke package of care. Please note, referrals for NHS or Local Authority funded services must come from a referring organisation.
