About this location

St Mark’s Court Care Home is located in a suburban area of Swansea, South Wales. Our home offers specialist nursing and dementia support for 38 adults, with 24-hour input from registered nurses. We can also offer end of life/palliative care and respite services.

Our goal at St Mark’s Court is to provide a safe, secure home for our residents, within a family-like environment. In addition to supporting adults with dementia, we can also assist those who have a range of other conditions, presentations and needs.

About our service

Our facilities and environment

St Mark’s Court is a large property, with the accommodation split across three floors, accessible via a lift. The friendly and welcoming decor helps residents feel at home. We offer a range of comfortable communal areas, which provide a relaxing atmosphere for the people who live here. This includes:

Residents also benefit from our tranquil garden areas, complete with:

  • Patio area with furniture

We also have specialised equipment within the home, such as a lift to all floors and a variety of hoists.

Our bedrooms

All of our bedrooms have been designed to provide maximum comfort to the people we support. The rooms are all furnished and homely, and can be personalised to individual tastes. Each bedroom has:

Our approach to support

Everybody who lives at St Mark’s Court has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive. This may include support in a number of areas, such as personal care, mobility, eating and drinking, accessing the community, and getting involved in activities.

Our home offers 24-hour nursing support and has a specialist dementia programme. We also have close links with multidisciplinary professionals, who help us ensure the health and wellbeing of the people we support. Our residents receive regular visits from GPs and dentists, and will also be supported to access other healthcare services, as required.

Activities

Everyone who lives at St Mark’s Court has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team, led by an activities co-ordinator, organise a variety of activities at the home, as well as out in the community. This might include activities such as:

Our support staff are highly experienced in supporting older people. Our in-house team consists of:

Our team also receive specialist staff training, designed to meet the needs of each individual we support. Our care team are supported by registered nurses on every shift, and they also receive diabetes management and specialist dementia support training.

How does home leave work?

We support family members to ensure our residents can attend home visits and events.

Can friends and families visit?

We have an open door policy. Family and friends are welcome to visit and spend time with their loved one.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

We work with family members to ensure that they are involved in their loved ones care. We will invite families to attend quarterly meetings, as well as arrange more regular catch ups as required, to discuss any key areas.

Will my loved one be able to have a phone or call me?

We have direct phone lines to all floors. We can also support residents with their own mobile phone should this be their preference.

What type of things are families expected to provide, and what is provided by the home?

Every bedroom will come with a bed, bedding and basic furniture. We expect families to provide room items such as additional décor or a television set to personalise their bedroom.

What are the bedrooms like?

All the rooms have their own washbasin facilities. Bedrooms come furnished, and residents are supported to personalise them to suit their needs and wishes.

Are external doors kept locked?

External doors and gates are locked, via a keypad system, however every person will be risk assessed which will detail the level of security needed to keep them safe.

What do residents eat and how do meal times work?

We have a dedicated catering team who provide personalised menus. Three daily meals are provided, plus snacks and drinks. Any preferences will be catered for; our menus will be adapted to meet dietary needs and wishes.

How does laundry work?

We have our own dedicated laundry team who ensure that residents always have access to clean clothes and bedding.

Is there anything they can’t bring or have?

We would ask that residents do not bring valuables, such as family heirlooms, as we would not want these to get lost.

Are pets allowed?

Unfortunately we don't allow pets to live in.

How do activities work?

Our dedicated activities co-ordinator organises a variety of activities, with input from residents. They will organise group activities, as well as more bespoke activities, tailored to the interests and preferences of each resident.

Do residents and families have an input into the care plans?

From assessment to post-admission, we encourage families to have full input in the development of personalised support plans for their loved one. We want the people we support to have a say in every aspect of the care they receive with us.

What are the car parking facilities?

We provide a small parking area to the rear of the property, plus there is on-street parking at the front of the property.

What is the smoking policy?

Residents can smoke in the designated smoking area. We do not permit smoking inside the building, and this includes vapes.

What are your fees and how are they funded?

Our fees can vary, and are based on an assessment of each individual's needs. We accept the following funding types: local authority funding, local authority funding and top up, NHS Continuing Healthcare (CHC) funding, and private/self-funding. Please note, referrals for NHS or local authority funded services must come from a referring organisation. We recommend contacting your local social care team to attain a referral.
