About this location

Charles Court Care Home is located in the historic market town of Hereford, in Herefordshire. Our home offers specialist nursing and dementia support for 76 adults, with 24-hour input from registered nurses. We can also offer end of life/palliative care and respite services.

Our goal at Charles Court is to provide personalised care, support, and outcomes to the people who live here, in a dignified manner. In addition to supporting adults with dementia, we can also assist those who have a multitude of other conditions, presentations and needs, including those with:

About our service

Our facilities and environment

Charles Court is a modern, purpose-built home. The detached home is split across two floors, with a lift providing access to both floors. This includes a specialist dementia unit which has feature walls and a wealth of further simulation.

The ground floor has 36 bedrooms, all with en-suite bathrooms, plus its own lounge, dining area, activity room and communal bathrooms. The ground floor also benefits from direct access to our beautiful communal gardens.

The first floor has an additional 40 bedrooms, all with en-suite bathrooms, as well as its own lounge, dining area, activity room, and communal bathrooms.

Our approach to support

Everybody who lives at Charles Court has their own person-centred care plan, which is co-produced with input from the individual, their family members and professionals involved in their care. We aim to help everyone in our care to make their own choices about the support they receive.

This may include support in a number of areas, such as personal care, mobility, eating and drinking, accessing the community, and social inclusion.

Nurses are on-site 24 hours per day, and we also have support from multidisciplinary professionals to ensure the wellbeing of the people who live here. The home has an on-site hairdresser, private physiotherapist, weekly GP ward rounds, access to 6-weekly visits from the podiatrist, and regular hearing and optician checks. In addition, all residents have access to the local dental team.

Activities

Everyone who lives at Charles Court has their own personalised activities planner and are encouraged to take part in activities which are meaningful and fulfilling to them. Our dedicated team organise a variety of activities at the home, as well as out in the community. This might include daily activities such as:

Exclusion profile

The home is unable to support people with complex behaviours that challenge, due to the vulnerability of the people supported within the home.

Family and friends FAQs

How does home leave work?

We support our residents to attend family events and visit home. We will work with the resident and their relatives to ensure risks are managed and that everyone is supported. We also provide a family room where we support families to share their special events with their loved ones.

Can friends and families visit?

We have an open door policy, and family and friends are welcome to visit and participate in activities at the service, seven days a week. Unfortunately we cannot cater for visitors to stay overnight. If families let us know in advance if you would like to eat with their loved one, and our catering team will make accommodations. We do have protected mealtimes to promote dignity.

Will I be involved and kept up to date with the progress of my loved one’s care and support?

We strive to keep the loved ones of our residents involved in their care. Regular relative meetings are arranged, where families will meet with the residents’ care team and hear about recent events at the home. We can also arrange more regular one-to-one catch-ups, either in person or via telephone, to ensure you are kept informed. In addition, the management team or a registered nurse will be available to assist you with queries at any time.

Will my loved one be able to have a phone or call me?

Charles Court has several incoming phone lines, and there are cordless phones at various points around the property which can be used by residents at any time. Each nursing floor carries its own mobile to facilitate easy contact. In addition, we can support our residents to get and use their own mobile phone, where appropriate.

What type of things are families expected to provide, and what is provided by the home?

Every bedroom will come with a bed, bedding and basic furniture such as a chest of drawers, wardrobe, and bedside table. In addition, daily meals and snacks are provided by our catering team, and our activities team will provide a range of activities at no cost. We expect families to provide toiletries and labelled clothing for their loved one, and room items such as additional décor or a television set. Additional services such as hairdressing, and external heath checks, will be funded by the resident or their family.

What are the bedrooms like?

All the rooms have their own en-suite facilities. Bedrooms come furnished, and residents are supported to personalise them to suit their needs and wishes. Some of the furniture has been purchased to suit the needs of the service specification and some of it is fitted, and therefore cannot be removed.

Are external doors kept locked?

External doors and gates are locked, via a keypad system, however every service user will be risk assessed which will detail the level of security needed, in line with their capacity, determined under the Mental Capacity Act. We have a large enclosed garden area and patio where individuals can freely exit and enter the building.

What do residents eat and how do meal times work?

We have a catering team who prepare meals three times a day, and snacks are available at all times. Residents are supported to choose what they want to eat at mealtimes, and there is always a choice available, as well as easy-read menus. Our team will also seek guidance from the resident, the family members, and nutritionists, to ensure that special dietary requirements are catered for.

How does laundry work?

Our team will ensure that residents always have access to clean clothes and bedding. We have industrial laundry facilities on-site, which our team will take care of on behalf of our residents.

Is there anything they can’t bring or have?

We would ask that residents do not bring valuables, such as family heirlooms and large amounts of cash to our home, as we would not want these to get lost. We may place restrictions on some items where the risks cannot be appropriately managed, such as scissors, medications, and razors.

Are pets allowed?

Unfortunately we don't allow pets to live in, but pets are welcome to visit.

How do activities work?

Every residents has an individual tailored activities planner. Our dedicated activities co-ordinator organises a variety of activities, with input from residents. This includes both in-house activities, including regular visiting external entertainers, as well as community engagement.

Do residents and families have an input into the care plans?

We work with the resident, the family, and social worker, to develop a life history and care plans, which are person-centred and in the best interest of the person. We use a variety of communication tools and work with health professionals to aid each resident in having input about every aspect of their care. Care plans are regularly reviewed, and family members will be asked for their input, along with invites to attend any care reviews and relatives meetings.

What are the car parking facilities?

Charles Court has a large, well lit car park where visitors are invited to park for free.

What is the smoking policy?

Residents can smoke in the designated smoking area in the enclosed back garden, which has a shelter. We do not permit smoking inside the building, and this includes vapes. We can facilitate the purchase of cigarettes and vapes. Families are also able to smoke in the designated areas outside of the home.

What are your fees and how are they funded?

Our fees can vary, and are based on an assessment of each individual's needs. We accept the following funding types: local authority funding, local authority funding and top up, NHS Continuing Healthcare (CHC) funding, and private/self-funding. Please note, referrals for NHS or local authority funded services must come from a referring organisation. We recommend contacting your local social care team to attain a referral.
